Blood Pressure and Longevity: The Silent Number That Ages Your Arteries

Blood pressure is the force your blood exerts against artery walls, written as two numbers: systolic (the push when the heart beats) over diastolic (the resting pressure between beats). It is one of the most measurable and most consequential markers of how you are aging — and, unusually, one you can often change. It also tends to be silent, which is exactly what makes it dangerous.

What the two numbers mean

The top number, systolic, is the pressure while the heart contracts; the bottom, diastolic, is the pressure while it rests between beats — both in millimetres of mercury (mmHg). Major guidelines now consider readings from 130/80 mmHg upward to be elevated (stage 1 hypertension), a threshold lowered in the 2017 ACC/AHA guideline to catch risk earlier (Whelton et al., 2018).

A single high reading is not a diagnosis — blood pressure varies with stress, caffeine, time of day and the 'white-coat' effect of a clinic. What matters is your usual pressure over time, which is why repeated measurements, including at home, give a truer picture than one visit.

Why it ages your arteries

Sustained high pressure damages the delicate lining of blood vessels and forces the heart to work harder, accelerating the stiffening and narrowing of arteries that underlies most cardiovascular disease. It is a leading driver of heart attack, stroke, heart failure, kidney disease and, increasingly recognised, cognitive decline.

The dose-response is steep and continuous. In a meta-analysis of one million adults, the risk of death from vascular causes roughly doubled for each 20 mmHg rise in systolic (or 10 mmHg in diastolic) pressure across a wide range, with no clear safe 'plateau' until quite low levels (Lewington et al., 2002).

Why it tracks with longevity

High systolic blood pressure is consistently ranked as the single leading modifiable risk factor for death worldwide, contributing to more deaths than smoking or high blood sugar (GBD 2019 Risk Factors Collaborators, 2020). Because it is common, silent and cumulative, it does a great deal of damage before anyone notices.

The encouraging corollary is that lowering it works. In the SPRINT trial, targeting a systolic pressure below 120 mmHg rather than below 140 reduced major cardiovascular events and all-cause mortality in higher-risk adults (SPRINT Research Group, 2015) — direct evidence that the number is not just a marker but a lever.

What actually moves it

Lifestyle changes have a real, measurable effect: regular physical activity, reaching or maintaining a healthy weight, moderating alcohol, and dietary pattern all matter. The DASH diet — rich in vegetables, fruit and low-fat dairy — combined with reduced sodium lowered blood pressure substantially in controlled trials (Sacks et al., 2001).

When lifestyle is not enough, effective and well-tolerated medications exist, and the benefit of treatment is well established. The practical first step is simply to know your number: a home blood-pressure monitor and a habit of occasional measurement turn an invisible risk into something you can track and act on.
