Grip Strength: A Simple Biomarker of Healthy Aging
Grip strength is exactly what it sounds like: how much force your hand can produce, measured with a handheld dynamometer. It costs almost nothing and takes seconds, yet in large studies it is one of the more reliable simple predictors of long-term health — a small window onto the strength and resilience of the whole body.
Why a hand squeeze says so much
Grip strength is a proxy for total muscle strength and, indirectly, muscle mass and quality. Because muscle reflects nutrition, physical activity and neuromuscular health all at once, a single grip reading integrates several things that matter for aging — which is why it tracks so well with frailty and sarcopenia (age-related muscle loss).
It is a marker, not a mechanism: a strong grip does not itself keep you healthy, it reports on the underlying strength that does.
What the research shows
In the international PURE study of nearly 140,000 adults, lower grip strength was associated with higher risk of death from any cause and of cardiovascular disease — and each 5 kg drop in grip carried a measurable increase in risk (Leong et al., 2015). Analyses of the large UK Biobank cohort found the same pattern for mortality, cardiovascular disease and even some cancers (Celis-Morales et al., 2018).
The association is consistent enough that grip strength is used clinically as part of assessing frailty — though, as always, it is a correlate, not a diagnosis.
How to measure it
The standard tool is a hand dynamometer: sit with the elbow at 90°, squeeze as hard as you can for a few seconds, take the best of two or three tries on each hand, and read the result in kilograms. Results are compared against reference ranges by age and sex.
As with other markers, the trend in your own number over time is more informative than a single reading — a steady decline is the signal worth acting on.
How to build it
Grip and overall strength respond to resistance training — working the major muscle groups, plus adequate protein and staying generally active — built up gradually and kept consistent. Improving whole-body strength tends to bring grip along with it.
